NYSAHA Parent Code of Conduct

Initiate a Parent Code of Conduct for your local association. Each listed parent or guardian will receive a secure email link to review and sign the document.

Association and player information

Choose the local association connected to this Parent Code. The completed document will be sent to the signing parent(s)/guardian(s). NYSAHA will also send a copy to the association when a completion recipient is configured.

After all required signatures are complete, the executed document will be emailed to each signing parent/guardian. If NYSAHA has a completion recipient on file for the selected association, that recipient will also receive a copy. Parents may forward their completed copy to their team or association contact when requested.
